Official Terms

The Road to 1,000 Promotion (the "Promotion") is a chance to help support the cholangiocarcinoma community.  The Promotion is subject to these Official Terms except as expressly stated otherwise. No purchase necessary to participate. The Promotion is organized by Citizen Health, c/o WeWork, 400 Concar Drive, San Mateo CA 94402 (the “Organizer”).

  1. Eligibility: The Promotion is open to legal residents of the United States who are at least eighteen (18) years of age or older as of the start of the Promotional Period and who have a cholangiocarcinoma diagnosis or are a caregiver of someone who has a cholangiocarcinoma diagnosis (“Participants”).   Employees of Citizen Health and their affiliates, Promotion supervisors and members of their immediate families, are not eligible to participate.  The Promotion is subject to all applicable federal, state and local laws, rules and regulations.  Void where prohibited.‍
  2. Promotional Period: The Promotion begins at 12:01 a.m. Pacific Time (PT) on March 20, 2025, and ends at 11:59 p.m. PT on April 9, 2025 (the “Promotional Period”), or when 100 Rewards have been claimed, whichever occurs first. Limit one Reward per natural person.‍
  3. How to Claim the Reward: Subject to the eligibility requirements, the Promotion is open to all patients and caregivers who sign up for Citizen Health’s free patient community during the Promotional Period by visiting the website www.citizen.health (or related urls) and completing the onboarding process in its entirety, including questions regarding the disease you have or your child has.  We encourage participants to consent to participate in research, however, this is optional and will not impact your ability to claim the reward.‍
  4. Rewards: The first 100 eligible participants who complete onboarding will receive a $49 Tango Gift Card. Rewards are distributed on a first-come, first-served basis. Once 100 gift cards have been awarded, the promotion will end. No substitution, transfer, or cash redemption of reward permitted, provided however that Organizer reserves the right to substitute a reward with another reward of equal or greater value should the advertised reward become unavailable for any reason, and no winner is entitled to any difference between the actual value and the retail value of any reward. Participants cannot assign or transfer their reward to another person or business.  Participants are solely responsible for any and all federal, state, and local taxes on reward. If reward notification or any reward is returned as undeliverable, or if Participants may not accept or receive a reward for any reason, reward will be forfeited.  Organizer will not make multiple attempts to contact Participants.
